Okay so apparently Sam is kidnapped by a pretty disturbing family - well it looks like a family, albeit a pretty fucked up one if that's really the case - and you have those snapshots of Sam in a cage and Dean bloody and tied to a chair yelling at the fucked up people that if they hurt his brother HE'LL KILL THEM ALL!
